Job Description

Department:            Community Services Division

FLSA:                Non-Exempt

Salary Range:        $30,000 - $33,000



GENERAL DESCRIPTION:

Performs routine (journey-level) case management work.  Work involves developing and maintaining long-term contact with clients, client families, and service providers for needed services.  May train others. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.



ORGANIZATIONAL RELATIONSHIPS:


1.    Immediate Supervisor:  AAA Supervisor

2.    Employees Supervised:  None

3.    Other:  Has regular contact with clients and their representatives, the general public and private agencies and organizations



EXAMPLES OF WORK:


Interviews clients or authorized representatives to collect necessary information, enter information into database, assess client needs, and make appropriate referrals



Provides ongoing case management and serves as a liaison between clients, client families, and service providers



Assists in identifying areas that are barriers to services for clients



Counsels clients and provides information and advocacy services relating to consumer problems, insurance, fraud, medical entitlements, housing, individual rights and other public benefits



Reviews, explains and assists clients in completing documents



Conducts data entry



Prepares BC reports as needed by management



Responds to and coordinates with HHS, TDI, ACL, SSA and CMS



Assists clients in preparing for hearings before an Administrative Law Judge and responds to requests for court action



Responsible for applying the Older Americans Act, the Texas Administrative Code and other AAA policy and procedure requirements



Coordinates and conducts outreach concerning BC programs 



Attends training events, conferences, seminars, and meetings as approved by the Community Services Director



May represent the agency on various committees and advisory boards



Performs other duties as assigned

Qualifications Education and Experience:
Graduation from an accredited four-year college or university with major course work in a relevant field. Experience and education may be substituted for one another.


Knowledge, Skills and Abilities:
Knowledge of program standards, objectives, methods and policies and procedures

Ability to analyze and prepare documents, reports and correspondence

Microsoft Office computer skills (Word, Excel, Outlook)

Ability to communicate effectively using correct grammar in written and oral forms


CERTIFICATES AND LICENSES REQUIRED
Valid Texas driverís license and reliable personal transportation

Earn Benefits Counseling I and II Certifications.
Compensation $30,000-$33,000
